This questionnaire has been compiled to assist in determining the capabilities of the business to supply goods and/or services to our current business. This information is required under current Health & Safety legislation in Australia – WHS Act 2011 and OHS Act Vic 2004). The information provided in this schedule will not relieve the business of the obligation to meet all the requirements of any contract that may eventuate this does not diminish the business’s responsibilities from any of its statutory obligations.
The purpose of this checklist is to facilitate the review of the contractor safety management requirements of our business. Where the answer to the question is ‘no’, you should take the required action. All questions must be answered. This checklist must be completed PRIOR to commencement of work. Once the business has been approved you will remain on our Approved Contractor’s List for a period of 12 months.
Please note as part of the organisation’s OH&S/WHS contractual arrangement and the completed Contractor OHS/WHS Systems Management Checklist, it is the organisation’s responsibility to inform this company, in writing, of any changes to these details. Therefore, to ensure all contractors and their employees are compliant with OH&S/WHS requirements, please forward any changes to our office within 7 days.
